Freedom at Home Dialysis Staff Assist Dialysis is focused on giving qualified ESRD patients another treatment option in cases where the patient is considered “non-admission” at any in-center facility. When ready for discharge, they can now be discharged to an SNF that is a clinical partner with our Staff-Assisted Dialysis program. They no longer have to endure extended hospital stays as our highly trained staff can see patients immediately upon discharge at the SNF receiving their dialysis treatment bedside. The program is transitional allowing the patient to return to the in-center when able.
